EXISTING MEMBERS REGISTRATION 2012/13
Please note that Registration Packages for 2012/13 for Existing Members will be mailed out during the week of June 1st, 2012.
NEW MEMBERS REGISTRATION 2012-2013
New members can register now by doing the following:
1-Download the below registration form from this site.
2-Fill all required information including full address, birth date, medicare number, e-mail address etc...
3-Sign the form by BOTH parent and player.
4-Provide proof of residency. A copy of a drivers license or a bill will be fine.
5-Make check payable to LMHF in the amount of:
